Gabapentin sits in medicine cabinets across Canada, prescribed for everything from nerve pain to seizures, and most people assume it is harmless. That assumption has a body count. A gabapentin overdose on its own rarely kills, but gabapentin combined with opioids, alcohol, or benzodiazepines has shown up in a growing share of overdose deaths nationwide. The gap between "safe medication" and "deadly combination" is smaller than the prescription label suggests.

How Likely Is a Gabapentin Overdose?
More likely than most people think. Gabapentin has a reputation as a low-risk medication. Doctors prescribe it freely, and it remains unscheduled at the federal level in both Canada and the United States. But "low risk" and "no risk" are not interchangeable.
Gabapentin can produce mild to moderate toxicity at doses as high as 35 to 40 grams, according to a clinical summary from the University of Minnesota College of Pharmacy1. For comparison, the max dose of gabapentin that doctors prescribe for seizures or neuropathic pain is 3,600 mg per day, split into three doses2. A 40-gram ingestion is more than ten times that ceiling. Animal studies conducted for the U.S. Food and Drug Administration found no lethal dose in mice or rats even at 8,000 mg per kilogram of body weight.
That wide safety margin explains why only two peer-reviewed case reports have documented death from gabapentin alone3. The real danger emerges when gabapentin enters the bloodstream alongside other central nervous system depressants. Opioids, benzodiazepines, and alcohol each slow breathing on their own. Add gabapentin to any of those, and the combined sedation can push respiratory depression past the point of recovery.
What Does a Gabapentin Overdose Look Like?
Gabapentin overdose symptoms follow a predictable pattern tied to how much the person took and what else is in their system. At lower overdose levels, the signs resemble an exaggerated version of the drug's normal side effects. At higher levels, they become medical emergencies.
Early warning signs include extreme drowsiness, slurred speech, and blurred or double vision. Coordination falls apart. Walking becomes unsteady, and fine motor tasks grow difficult. Nausea, vomiting, and diarrhoea round out the gastrointestinal picture.
As gabapentin toxicity worsens, cardiovascular symptoms appear. Heart rate climbs (tachycardia) and blood pressure drops. Breathing becomes laboured and shallow. In people with impaired kidney function, the drug can accumulate faster than the body clears it, and coma becomes a real possibility3.

One of the trickiest aspects of gabapentin overdose is timing. The drug follows a saturable absorption pathway, meaning the gut can only absorb so much at once2. Peak blood levels may not arrive for three to four hours after ingestion. Someone who appears stable in the first hour can deteriorate rapidly as delayed absorption catches up.
Why Is Gabapentin So Much More Dangerous With Other Drugs?

Gabapentin alone has a wide safety margin. Gabapentin mixed with opioids does not. That distinction matters because a staggering number of people take both at the same time.
A landmark Ontario study published in PLOS Medicine tracked opioid users across the province from 1997 to 20134. The research team found that people prescribed both gabapentin and opioids had a 49% higher risk of dying from an opioid-related overdose compared to those prescribed opioids alone. At moderate and high gabapentin doses, that figure climbed to nearly 60%. Among gabapentin users in the study, 46% also held a concurrent opioid prescription.
A 2022 CDC analysis of overdose data from 23 U.S. states confirmed the pattern on a national scale5. Of 58,362 overdose deaths with toxicology results, 9.7% tested positive for gabapentin. Among those gabapentin-positive deaths, 85% to 90% also involved an opioid. The proportion of gabapentin-involved deaths linked to illicitly manufactured fentanyl rose from 56.8% in early 2019 to 69.2% by late 2020.
These numbers paint a grim picture. Gabapentin and opioids both depress the central nervous system through separate pathways, and their combined effect on breathing is greater than either drug produces alone. Gabapentin may also increase opioid bioavailability, amplifying sedation beyond what the opioid dose would predict. Alcohol and benzodiazepines create a similar layering effect, and people who combine all three with gabapentin face the steepest risk of all.
How Do Doctors Treat a Gabapentin Overdose?
No antidote exists for gabapentin. That single fact shapes every treatment decision that follows. Gabapentin overdose treatment is entirely supportive, focused on keeping the person alive and stable until the body clears the drug on its own6.
Emergency teams begin with airway management. If breathing has slowed or stopped, supplemental oxygen or mechanical ventilation keeps oxygen flowing to the brain. Intravenous fluids maintain blood pressure and hydration. Continuous cardiac monitoring tracks heart rhythm for dangerous irregularities.
If the person reaches the hospital within a narrow window after ingestion, activated charcoal may reduce further absorption from the gastrointestinal tract. Beyond that window, the focus turns to elimination. Gabapentin is cleared almost entirely by the kidneys, which means healthy kidneys will flush the drug without intervention over the course of hours. For patients with kidney impairment, or in cases of severe toxicity, haemodialysis can remove gabapentin from the bloodstream directly6.
When opioids are part of the picture, naloxone can reverse the opioid component of the overdose. Naloxone does nothing for gabapentin itself, but it can restore breathing that opioids have suppressed. That intervention alone has saved countless lives in mixed-drug overdoses involving gabapentin.

Who Is Most Likely to Misuse Gabapentin?
Gabapentin misuse does not look like the stereotype of street-drug addiction. Most people who misuse the drug already have a prescription. They take more than prescribed, refill early, or combine it with other substances to heighten the effect.
A 2016 systematic review published in Addiction estimated that 15% to 22% of people who misuse opioids also misuse gabapentin7. In the general population, that rate was approximately 1%. A separate study of 250 individuals in a correctional facility found that 26% of those with an opioid use disorder reported gabapentin misuse, compared to just 4% of those without one8.
People with kidney disease face a different kind of risk. Because gabapentin exits the body unchanged through the kidneys, any reduction in kidney function slows clearance and allows the drug to build up in the bloodstream. Elderly patients are especially vulnerable because kidney function declines naturally with age, making accumulation-related toxicity more likely even at standard doses2.
The pattern of gabapentin withdrawal also drives continued misuse. Stopping the drug abruptly can trigger seizures, insomnia, and intense anxiety. Those withdrawal symptoms push people back toward the medication, creating a cycle that deepens dependence over time.
What Makes Gabapentin Overdose Harder to Detect?

Gabapentin falls through the cracks of standard drug screening. Most hospital toxicology panels do not test for it, which means emergency physicians may not know the drug is involved unless the patient or a bystander provides that information. In overdose situations where the person is unconscious, gabapentin overdose symptoms can be mistaken for the effects of other sedatives, and that gap in testing can delay the correct treatment response.
The drug's non-linear absorption adds another layer of unpredictability. At a dose of 900 mg per day, about 60% of gabapentin gets absorbed. At 3,600 mg per day, that drops to roughly 33%9. Doubling the dose does not double the blood level, which can create a false sense of safety. But if someone takes repeated large doses over a short period, each round of absorption stacks on top of the last, producing a delayed spike that arrives hours after the final dose.
U.S. Poison Control data documented a 4.3-fold increase in gabapentin and pregabalin abuse reports between 2006 and 201410. Some U.S. states have responded by classifying gabapentin as a controlled substance. Canada has not taken that step, and gabapentin remains available with a standard prescription.

Does naloxone work for a gabapentin overdose?
Naloxone reverses opioid effects only and will not counteract gabapentin on its own. In a mixed overdose involving both gabapentin and an opioid, naloxone can restore breathing that the opioid has suppressed, which may be enough to prevent death. Gabapentin itself has no specific antidote.
